Sky News can reveal that 75 people with a learning disability or autism have died whilst admitted to inpatient units in England since 2015 – 35 deaths were in the last three years.
An independent review into one of those deaths, 47 year old Clive Treacey, has concluded that his death was ‘potentially avoidable’.
According to the review Clive experienced an ‘unnecessarily prolonged detention in hospital of nearly 10 years’, and there are unanswered questions about the night he died.
His family are now pushing for a second inquest.
